I have one form on my account called Coaching Assessment, however when I tested out the form, it emailed me the "client intake" form, which was a template I used to make the coaching assessment, however, I don't know why that's being sent instead. Can you help?

Have you cloned this form?
Please be noted that when you edit your form's notification email alert, any changes you make in your form (add/remove questions) will not be reflected in your email template. You will have to make the changes manually. The form which you cloned must have the email alert edited and that is the reason it still has the old question which is not part of your form anymore.
To solve this problem you should delete your form's notification email alert and add a new one. When you add a new notification alert, the email template will have the all the questions of your form.

However the widget I also added to the form for my clients to take a photo
of themselves to submit, doesn't show up on the email that prompts me when
they've filled it out - can you help?

Do you mean to say that you do not receive the image taken from the "Take Photo" widget in your submission emails?
I did check your form's notification email alert and it seems to be configured correctly and you should be receiving the image in your submission emails. I cloned your form and sent a test submission. The submission email which I received does display the image correctly. Please check the screenshot below:
I would suggest you to please test it once again and get back to us if the issue persists.
