I can see that your email notification was set to email@example.com. I've checked our log to make sure your email is not included on our bounce list. I've also checked our email logs and I can see that you should be receiving email notifications. Have you checked your Spam/Junk folder?
I would also suggest you contact your email provider to whitelist Jotform's Domains and IP Addresses to make sure your email server does not reject emails from Jotform.
Please let us know if you need further help.
I have checked the spam/junk folder and it is not there.
Thanks for updating us here.
The recipient email address of the form embedded here http://worshipradionetwork.org/Worship-FM/pledge.html is "firstname.lastname@example.org" and not the one you mentioned on your initial post. Is that the form in question? Please review your recipient email, here's a guide on how to change your form notification recipient emails. http://www.jotform.com/help/41-Finding-out-E-mail-Address-used-for-submission-notifications
