Need Products and Donation options on the same form

Our non-profit site has 15 differnet books (products) that we give away in any quantity. We have a suggested donation amount of $3/ea that is the same for all of them.We are now attempting to set up taking donations online by using the "Payment Options", but I can't figure out how to combine the "multi-product" option (found in the for-profit section) with the "choose a donation amount" option in the non-profit section. (NOTE: We don't need an amount tied to each item, we just need "Suggest donation of $3/each" stated once on the page.)

- The page needs to show a thumbnail of each product (linked to an enlargement);
- A custom quantity box for the customer to enter the amount (often they order 100's of if different titles);
- Then enter a donation amount or opt out of donation;
- Finally, check out with our Authorize.net account.

The available options in JotForm are "either/or"and I can't combine my needs. I need customers to be able to (1) choose quantities of books and (2) donate any amount in the same form instead of separately.

This is not currently possible to combine different payment types, but we can try some other options.

I have used products with 0 price and custom quantity options, but the last product has been set to $1 price, so that user can enter a custom amount.

Also, a Product Lightbox added to the form to add the clickable product images.

You can clone mentioned above form to your account.

Alternatively, it is possible to use your custom fields to calculate the suggested amount and forward it to the changeable Donation field. In this case, your products will be outside of the Authorize.Net tool, and the lightbox option will not work.
