Can you describe exactly what happens when you try to upload a file? Do you have this form embedded somewhere or are you using it as standalone?
I checked the submissions page for the upload form in your account and found that there were files uploaded to it, so I don't understand what the exact problem is.

When you say "destination box", do you mean a Dropbox folder? If the answer is yes, then you'd have to integrate your form to Dropbox first so that uploaded files will be sent to your Dropbox account. Click the link below to learn how to integrate Dropbox.
