How to find a lost form?

    Aloha, I spent two days working on making forms and then a half hour ago I tried to fill one out on my website and when I hit send I got an error saying there was no form. I can see the form on my web page here: http://www.islandexpresswebdesign.com/affluent/contact.html   but it does not show up in My Forms any more in JotForm.  On my website this was the iFrame I embedded:

    What did I do wrong to lose this form?  I called it Affluent, but I had cloned it from a previous form called Affluent Creative and had made it smaller and changed a few other things.

    We received the information that this situation is solved now. This situation  was caused by a problem with data replication .

    I checked your account and could see the form there.
