I have followed your Clinchpad integrations notes exactly.
The Lead name is copying over to clinch pad; as is the "Customer questions field" but none of the other requested fields are doing so. I have even edited the field in jotform to copy the title exactly.

Some of the fields are indicated as REQUIRED in JotForm, could this be causing the issue?? I have even attempted using the FieldID without any luck.

I've tested this further to try to lend my colleague a hand with it. I found that the integration and webhook shined through rather smoothly for it. However, It looks to be limited or picky with the fields that are mappable from your Form to your ClinchPad Configuration.
In my first test I had mapped out the fields appropriately similar to how you saw in our provided guide. I tried this with and without a requirement, but that seemed to have no effect on it. It was a test which was very similar to your form. I mostly mapped the first try with the different field types just to test it further and see what would happen.
FIRST TRY MAPPING (UNEXACT):
However, I then created a second test with another new form specifically mapped to match most of the information inside of clinch and map them to the same exact matching fields from JotForm. The results were a bit more realistic, but some of the data still got left out due to the fact it's limited.
