I cannot replicate this problem on my end. Are you having this problem when creating a new form or modifying an existing one? Can you give me information on the browser which you are using? Also if you have not done so already please try clearing your browser cache to see if that fixes the problem.
I'm using Google Chrome. It happens when I modify and create new. I will try clearing my cache to see if that works.
