No Submissions Are Saved When Submitted

I've cloned and imported the file from my old account to my new account. Clone form works but i am still not receiving submissions/answers from the form. 

The form itselfs works (i've tested it plenty of times) but I'm not recording any of the survey information for me to pull up a report. The survey answers are not in my inbox and there's no report. Basically after you fill out the form the answers from the form are not in my inbox. 

I do see you have embedded the form to a php page and then embed that php page to your site using iFrame. This may have broken functionality/contact to our service.

Is it possible for you to embed the form to your website using our hosted form using the iFrame Method instead?
