Why does my form change?

Hello,

I created and saved a form a few days ago. When I return to it or when someone tries to use it, some of my changes have reverted or the format has changed.

I saved the form after creation and tested it myself. All the changes had saved. But a few hours later some of these changes had disappeared - including the form url and the email address to which the form sends notifications!

This has happened multiple times.
