Hello,
I would like to know if it is possible to attach a signature to a code or emplyee number in a form. I would like to create a form that I can have my emplyees just puch in their code and then have their signature come up rather then have them sign. I also will need to have many signatures on the same form.
Please let me know of somehitng of this nature is possible within Jot Form.

Hello Chantel,
It can be possible to show a specific image (signature) after a matching code entered using hide/show conditions (This user guide can explain more of the overview of the feature).
But to be able to help you further and guide you better, would it be fine for you to share with us the use of this form such as, will it just accept submissions? I'm looking at a consideration that what you're looking for may be possible beyond what JotForm is designed to provide.

Thank you for your response, ideally the form would be used as a hazard inspection. So what happens is everyday before work the employees or contract visitors have to sign off on a hazard meeting form. We are trying to use less paper and become more technical. It would be time efficient if each employee was assigned a employee number in which they would enter into the form daily to submit their signature. The form would have to be able to accept up to at least 20 signatures or code enters rather. The form would also have to have the option for the e signature for visitors that do not have a pre installed code signature.
Now taking this a step further it would be optimal if the form once submitted could stay open for the duration of the day in case other visitors arrive on site.
I hope I was able to effectively put my thoughts into words.

Yes - this is possible. We are going to use this principle to implement it
You can implement the workflow as shown below:
1. Create a form which has a code section, and a signature section:
2. Then create rules that would determine when the signature section will be shown:
3. Then, create another show/hide conditional logic rule to determine what kind of signature will be used:

Thank you for showing this to me; however could we take this a step further and have lets say 16 employees enter their number into the same number box and have each of their saved signatures appear on the form?

Hi Chantel,
You can update/modify the suggested method of our colleague TitusN to accomodate the 16 different employees to fill-in the form.

Hello,
Thank you for your assistance.
This form needs to be signed by everyone present in the meeting before being submitted. There signatures must appear on the same sheet.
Rather then have everyone sign on the pad, we thought a signature attached to their code would be more efficient.
Is it possible to enter the code without the form submitting each time since each signature needs to appear on one form?

How exactly do you want the signatures to appear (as an image/as a link)? And where would the filled form be accessed (email/spreadsheet) by your organization?

Hello,
Ideally the signatures would appear as an image. Once the form is completed it would be sent out via email to someone in the office who would then file it.
