Hi!
I have one problem with designing my own survey.
In my survey, I want to ask one question with check boxes. However, I can't create additional fillable field next to (or below) this check box. Is that even possible or not? Below you have an example of what I mean.
QUESTION: Which medical condition do you presently have?
ANSWER #1 (checkbox): celiac disease
ANSWER #2 (checkbox): low back pain
ANSWER #3 (checkbox): hypertensionIf somebody checks a checkbox for "hypertension", I wanted him to fill additional field (which I want to be next or below "hypertension checkbox") where she/he tells me the numbers of lower and upper blood pressure. But as far as I know, this is not possible. Am I right?

Hello Simon,
You can add one "Textbox" field and add condition to show/hide this textbox if the hypertension is selected.
Please take a look at the following demo form which I have created: http://form.jotformpro.com/form/41421981736962? If you select "Hypertension" checkbox, you will see a textbox appears next to the question. I added the following condition:
