My forms have been disabled. Help?

My forms have all been disabled. I have used the words username and password on one of the forms which I know it's not allow and have removed it since, so i just want my forms back enabled.

I believe you already know about phishing.So, to avoaid auto-suspend please do not use fields like password.

I have re-activated your account and your Forms are enabled now.
