How do I add a subheading to a category?

If you meant you want to have a sub-category text in your form http://www.jotform.us/form/41445506095151 checkbox items, I think it is not possible with the current regular Checkbox field

What you can do instead is just create a group of Checkbox in the form and use the Header field(in Form tools) as the category text.

Something like this

Category 1(Header field)

          Checkbox (sub-category)

                               Option 1

                               Option 2

                               Option 3

Category 2(Header field)

          Checkbox (sub-category)

                               Option 1

                               Option 2

                               Option 3
