My name is Chris Edgerly and I'm the President of Edgy Brothers, a small design company out of Los Angeles. I have a few questions about your product and whether or not it would be a fit for our company.
We've created one successful Kickstarter campaign already (for a custom deck of playing cards, with other add on items included) and are going to be doing up to four campaigns a year. Our challenge is in dealing with the very large number of backers that a campaign can send our way. In our first campaign alone we had 1600 backers, and all of them ordered specific items from our campaign, from various decks of cards with different design themes to coins to dice to t-shirts, etc. In order to effectively fulfill their orders we've enlisted the services of Backerkit.com, a company that specializes in fulfilling crowdfunding campaigns like those on Kickstarter. They provide a survey form that backers fill out specifying the different items they ordered, the number of each kind of item, as well as allowing them to purchase more items if they wish, adding to their Kickstarter pledge total via credit card. It's very effective, complete with a manager page with a dashboard and analysis for survey creators, but Backerkit charges a subtsantial fee and takes a percentage of all funds raised using their survey. Plus, we want our backers to eventually come to our wep page after a campaign has ended to see other items we have available for purchase, and as of now we've got them bouncing around between Kickstarter, Backerkit and our own site. It's time to see about simplifying.
We'd like to to do a similar kind of survey on our site using your product. We need one that we can replace regularly when we close one campaign and launch a new one. Our site is built on i-web and can handle Jotform if it's to be installed via html snippets, etc.. Here's what we'd need our survey to do:
Record specific products each backer ordered, keep a running tally of their cost, and allow them to add new items to their order using a credit card. It also has to manage all the addresses, names and emails of each backer and keep them in some sort of spreadsheet form for us to access whenever necessary.
Is this something your product can provide? If so, we'd be very interested. There are likely other functions we'd need as well but for now this is a good start to see if we're compatible. Please give me a shout and let me know your thoughts.

If I understand your requirement correctly, you are looking for following functionality:

2. Process payment using credit card.
