How can I add a signature line to my existing form

Would like to edit my existing form to add a signature and a box to check in lieu of signature if filing form online? Thanks, Bill

You can use the e-signature widget on your form then add a checkbox they can check if they are filling it out online. See example below:

You can also set a conditional logic to only show the e-signature field if the checkbox is checked.
