I duplicated my foster applicaiton form and changed a bunch of the questions to make it an adoption form. it appears fine on our web site, fills in and submits fine, but comes to our email as the old foster form! not sure why. tried removing it, copying the html code again, embedding again, and still an issue. please help very important and i'm lost

It is also necessary to edit the email notification. Perhaps the best solution is to delete the old one:

And to create a new one.

Please let us know if the problem persists.

    Thank you

That is not an option. I want to keep BOTH FORMS. Why would editing the email notification make a difference? The form shows correctly but then i fill it in and it is somehow the other form that I used as a template for this new form (which is so similar I do not want to start from scratch).

One is a foster applicaiton, one is an adoption applicaiton. Am I not going to be able to use two jot forms on our web page???

    Hello handsofhopedana,

If I understand your question correctly, the cloned form still send you submission email with the old template question. Is that correct?

Please be noted that when you edit your form's notification email alert it becomes dirty. That means any changes you make to your form will not be reflecting in your notification email template. You will have to manually make the changes in the email template as well.

You must have the email alert edited of your main form and when you cloned the same email alert must have been copied to cloned form as well. Since it was already edited, the changes you made to your cloned form are still not reflecting.

You do not have to delete form, you need to just delete the notification email alert of the cloned form and add a new email alert. Following guides should help you:

    Delete existing email alert:  http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder

    Add new email alert:  http://www.jotform.com/help/25-Setting-Up-Email-Notifications

    Hope this helps.

Do get back to us if you have any questions.

    Thank you!
