How do I create a link in a document that sends them to my form and then have the information sent to my email?

    Hello white5housecaw,

    It seems you need add a hyperlink in your document and on click of that you want your user to be redirected to your form. Is that correct?

    You need to simply grab your form's direct URL and add it in your form and create link. here are the steps to grab your form's direct URL:

    1. Load your form in editor.

    2. Select "Setup & Embed" tab from the toolbar.

    3. Click on "Embed Form" and copy the URL. Please check the screenshot below:

    You can use this URL to create a link in your document.

    Now coming to your second question on the having the information sent to your email, you need to add a form notification email alert in your form. That will send the submission email to you. The following guide should help you:  https://www.jotform.com/help/25-Setting-Up-Email-Notifications

    If you need to change the email address which is used to receive submission email, you should change the "Recipient Email" address of your email alert. The following guide should help you:  https://www.jotform.com/help/41-Finding-out-E-mail-Address-used-for-submission-notifications
