Hi I upgraded my account today and nI just got complains from customers that claiming the forms are NOT working.
I loged in to my account here and its shows that the Form is disabled. please see details below:

I did check your payment details and found that you have not mentioned your username while making the subscription payment.
I have taken care of this issue and upgraded your account to premium monthly subscription.
