PDF convert

    To whom this may concern,

           I am needing the forms that were emailed by Michael Faulkner LCSW to be in a PDF format so I do not have to print, scan, and then download.  They need to be able to convert to PDF when I hit print so all I have to do is save then download.

    I could not find forms associated with the therapy123 account. If you share with us the form's url it's easier for us to provide the correct support.
    To download a pdf version of the form, please go to Myforms> select the preferred form > Click submissions:

    Then select the submission and click "Get PDF":
