Upon checking your account, I found that you still have a guest account. Since we cannot check forms of guest account, I would suggest you to sign up for a free JotForm account using your email address "email@example.com". This will ensure that your form is moved to your new account.
If you do not see your form in your account, do get back to us with your new username and we will manually move the form.
Once your form is in your free account, we will take a look on why it is not sending submission emails. But this is strange that it sends submission email from iPad but does not work in laptop.
