I keep getting an error message when trying to submit a form I've used before

    Sorry! Only one entry is allowed.
    Multiple submissions are disabled for this form.
    I have used the report twice prior.  It shows no limit to subissions

    I cloned your form and I can see that unique submission is set to Loose Check. If set to loose check, you won't be able to submit twice on the same computer. to fix this problem, you can set it to No Check.

    Once done, Save your form and try again.
