Populate Fields

I want few fields to populate depending on entry on the first field. I am not a Mysql or PHP expert and need help in getting this work. VERY URGENT

I uploaded the data in Mysql and I will have to host it to php enable webserver which i will do shortly. (Any recommendation on hosting services ?)

Am I correct in assuming that the data you will be populating to some fields in your form will come from an external database? If that's the case, the only way to achieve that is by writing a custom script that will be integrated to your form's source code which will need to be hosted on your own server.

I would suggest hiring a web developer to help you with this custom solution. Odesk and Elance are some of the few sites where you can get a person to do this job.
