How to add a defined list of items that a customer can indicate qty required

    Hi, I currently use Jotform for our customer order form at this page:

    I would like to offer our customers an additional order form option for one particular range of products. So rather than having the last row as per the above form ("Please supply", with "add row" functionality) I would like to list a finite range of product codes, descriptions and price, so that the customer can nominate the quantity they require for each item. Then the customer can submit that selection as an order. 

    Could you please advise how I can make this work? I've tried a number of options and widgets but have not been able to figure it out myself :-/

    How would you like adding a Purchase Order control to your form wherein you can define products and set a quantity option?
