    Unfortunately, we don't have any phone support but we can still help you via forum. About changing credit card it would be best if you fully cancel your existing subscription then re-subscribe under http://www.jotform.com/myaccount/billing using your new credit card then let us know once you have fully cancelled and renewed your subscription so that we can verify if the subscription was cancelled from our end to avoid duplicate charge.
