I have just installed a form onto my web domain, however, the form is not passing critical information (specifically the name field)

The field variable on your form email notification is incorrect. It's most probably not updating anymore after you made changes on the notification. However, you can update it manually by clicking the field names found on the side bar just like the following screenshot.
Setup & Embed -> Email Alerts -> Open Notification
Or, to ensure that all field variables are correct. You can delete the email notification and create a new one.
