There is a ghost form attached to ours, how do we get rid of it? and is that whats causing us to reach our quota?

We created the first form ( nomination form) then we created the ballot form, it seems that there is a ghost form attached to our ballot form it reads IBCM Awards 2014, but we saved our form; Juneteenth Urban Music Award Ballot. What is this and is this causing us to go over our quota?

if you go to http://www.memphisjuneteenth.com and go to the award show tab at top of ticket, then scroll down to vote you'll see our correct form but when it shows up in our inbox it states whats listed above. I tried resaving to make sure everything was right.

Sorry but I don't get what you mean by this "when it shows up in our inbox it states whats listed above". I checked your site and I can see the correct form. When you say ghost form are you saying you are getting duplicate submissions?

In your Vote button you don't use any embed code, you only used the direct URL which I don't think will conflict with your code since once user clicked on Vote they will be redirected to the form URL.
