Formatting question: How to allow multiple selection on Radio Buttons

    Hello,

    It took two days to create my form and I just tested it - OH NO! Is there a way for people to choose multiple options on a single radial dial field? (It is the ONLY form in my account - please view it) https://secure.jotform.us/form/41535253695156

    The form was getting so long that I went back and put all the "Conditions and Complaints" into one field. When I tested the form, it will only allow me to chose one "complaint" rather than seveal. This is a significant problem for me.

    Is there a way to allow multiple entries without having to re-write that section with 281 conditions?! Please say yes.

    Thank you for contacting us.

    There is no way to allow multiple selection for Radio Button fields, so you will need to use Check Boxes in this case. It is possible to copy options (answers) of the Radio Buttons and paste them into the Check Box options.
