How do I create drop down answers to items users select?

I am trying to create a form which will be quite complrehensive and built around an exercise consultation. Selecting an item on the form should ideal result in a drop down answer... Our instructors ask clients a question, their response dictates which item we select and thus the 'drop down' answer. The form is submitted and we print the drop down answers and give it to clients. Is this possible?

Yes it is, add the Drop Down field from Form Tools, select the field, and click on "Options":
