How can I have blank spaces on a check box area to creat columns?

I tried just putting in additional returns without any wording, but what I get are checked check boxes with a blank input.  I'm wanting to maintain my column structure, however.   (See screen shot below and notice the checked boxes.)  Any thoughts?

    Thanks!!

    Thanks

Okay - I'll work on the form and see if it will offer the functionality that I would be looking for and knowing you can fix this problem, I'll see if it will be a good fit for the project I have in mind.

    Thanks so much,

    Veronica
