I was sent an HTML attachment, which uses jotform.com to send the spammers victims personal details.

Thank you for bringing this to our attention. We have 3 ways to curb phishing activities in JotForm, automated process by our system, manual process by real human beings, and community involvement (reports by the public).
Do you know the URL where the suspecteed phishing form is currently embedded? Unfortunately, the acreenshot you sent did not appear on the web page. Kindly send it again along with the URL so that we can identify which form and user account to suspend.
