We cannot log on to the service to view our forms

    We had been using Jotform for business forms for the Candlewood Lake Authority.  We were almost at the form capacity so we upgraded to the premium membership.  On the upgrade form, we changed the log on name from "ring003" to "CLA01" but we can't seem to log in to use the website.

    What should we do?

    Hi Mark,

    Thank you for your message. We tried to locate CLA01 in our member database but failed to find a match. This means that your username remains as ring003 for now. Note that you cannot actually change your username yourself, you have to contact us to make the change for you. We can proceed with your permission, but for now I have sent the instructions on how to reset your password to your registered email address (ring003@xxx.xxx).

    We'll await your response so that we can change your username.
