Upload Error

    I'm getting an upload error and not sure what needs to be done to fix it.

    Please provide us more details about the upload error you encountered.

    I made a test submission using your form in your website  http://www.swurvradio.com/hookup/

    and I was able to upload the cover and mp3 song successfully as I was able to see the Thank You page

    If issues persist, please try also re-embedding the form using its iframe embed code instead of the script code
