Forms do not have form title.

    Before the update you provided to our forms, we were able to pull up a submission transer it to PDF form and print. For instance, when we would print a USA form submission, the PDF would show "USA Mens Comp Order Form" on the top of the paticular order.

    Now, when we print the submission, it shows the form, but not the form title.  Why is this?  We need to have the form title at the top of all form submissions, so we now which customer's form we are looking at?

    Does this make sense?  We are starting our new season and this will be the second question from us, with issues we are having with our forms.  Please advise asap.

    Hello Mike,

    I'm sorry for the trouble caused to you.

    I did try to download your form's submission in PDF and I am unable to replicate the issue you are having. The exported PDF does display the form title. Please check the screenshot below:

    It seems to be working as expected. I would suggest you to test it again and get back to us if the issue persists.
