Is there a reason you allow identity thieves to use your forms?

    This form of yours is having people enter their personal information and credit/debit card information to be EXPLOITED. They use the card information to make fraudulent purchases online. If this could be dealt with in any way, that would be greatly appreciated.

    Hi Brianna,

    We are sincerely sorry for this inconvenience but thank you for bringing this to our attention. I have already suspended the account responsible for this form.

    If this can help, the last IP address used by the culprit to log in is 98.248.145.159. We have an anti-phishing system in place that's working 24/7, suspending forms found to be used in illegal purposes although I am not quite sure why this form has evaded our phishing detector.
