I edited the form and am not receiving notifications

    I changed the 2014 Strong Tower Ranch Volunteer Application form, a field (Evangelism Training date), & saved the form, deleted the notification and auto responder & recreated, but when i edit a submission, it shows the hidden fields and a duplicate set with no boxes. I did two test applications/forms for which I didn't receive a notification in my email. I did receive the autoresponder email for the one that I included my real email. Help?

    I created a copy of your form and made a test submission. We have the information the notification was sent:

    To hide empty fields in the email you can change the settings under Preferences:
    Regarding the Auto-responder I could see you did some editing. Even though it is possible to customize auto-responders/notifications, if any change is made to the form (adding/remove fields), the auto-responders/notifications don't update it's structure.
    So it is necessary to create a new one.
