Why do I receive the a "the should be empty" box at the end of my form?

    When I embed my form into an email, the formatting changes.  Bullets are included, spacing is off, and the submit button doesn't work sometimes.

    Hi,

    Thank you for contacting us.

    First off, I have to clarify to you that you cannot really embed the form within an email message. You can only use the form's direct URL instead as link in your email message.

    For example, you can click on this URL link http://www.jotform.com/41633621494151 to access your form.

    The form embed process will require the webpage to have the html container/tags where the form can be embedded. You may want to review this JotForm user guide for more details

    Addition:

    I also test submitted your form http://www.jotform.com/41633621494151 and was successful. I did not encounter any error message upon submission.
