Hi,
I am working on this form http://form.jotform.co/form/33464740042852
There is a lot of conditional logic in this long form, and while most of it is working, the "skip page" feature seems to be problematic.
the first question on the form asks What type of referral are you requesting. Then it gives 3 options (outpatient ultrasound, digital radiograph submission, analog radiograph submission).
Depending on which you select, after you fill out all the information on page 1, on clicking next you should be taken to page 2, 3, or 4, respectively. At the moment, no matter which of the 3 choices you select you are redirected to page 2 AND either page 3 or 4 relative to which ever item you selected.
e.g.
i select digital radiograph.
On clicking "next" at the end of page 1, it SHOULD take me to page 3. And the conditional logic is in place for it to do so.
However, when I click next it loads BOTH page 2 and 3 simultaneously.
This issue is consistent and I have replicated it in all browswers (google chrome, firefox & IE).
Can you please help with this? I cannot figure out what can possibly be wrong or why it is behaving this way. When we created the file & deployed it, it was working great. In late May I was notified of this issue.

Hi Sommer,
There must be an element in your form that is breaking the skip to page 3 condition.
I made a test form on my end by adding only that single condition
but it still is not working on the test form http://www.jotform.me/form/41636687684470
Let me investigate this further. I will come back to you as soon as I found out a fix for it.

Hi Sommer,
I was able to find out the caused of issue.
It was the Date Picker and IoS7 Time widget in the form
Unfortunately, this 2 widget were causing conflict with the condition statement on the form.
You will need to remove them and use an alternative field (I suggest the regular Date/Time field).
Here is a clone of your form where the only change I did was removed the mentioned widget, and the condition logic on the form now work as intended.

Thanks it works perfectly! I have cloned the form & subbed out the two widgets.
