can we pdf output include text and headers

Dear Jotform

I have created a form using your great system which I am happy with. It is the only one on my account.

Automatically  sending the "form filler" a  link to the pdf of the form works well however the pdf only includes the questions and his answers. 

I would like to include the fixed text and headings.

Can you please tell me how I can do his (if I can)

I enclose the example out put from the this form as sent by your system

Thank you for contacting us.

You can actually check the option to Show Headers and Texts. While on submissions page, click on the small wrench icon on the upper right hand corner. Put a check on Show Headers and Texts. See image below:

My apologies, I overlook the "email" on your question. The above instruction only works when downloading the pdf file from the submissions page. It is not applicable to email notification.
