Cannot add form to facebook fan page

Hello,
After preparing the form on jotform I clicked the facebook tab to connect and put the codes on my facebook fan page but after clicking facebook icon it appears a small and white page. It doesn't connect to facebook to continue. Need advise.

Hi,

Can you please share to us your Facebook URL page where we can at least test the form.

We will be able to provide you with better suggestion response after we have seen the actual form on the FB page.

We will wait for your response.
