Notification emails stopped working

I just realized that we have been missing all quote requests that are generated by our jot form for over one month (since 5/7/14). This function was working properly for many many months prior to this. What has gone wrong? This is caused us to miss a lot of business.

I checked our mail log and we have the information of an email being sent today:

Have you checked your Spam folder? Also you can add a custom sender using Mandrill as some email providers block emails from noreply/noresponse email addresses. Please let us know if the problem persists.
