Recently several order forms from my website have ben coming up blank.
I've been forced to contact my customers (after obtaining their e-mail from the paypal payment) and ask them to provide the order information again via e-mail, which causes them great annoyance.
I have attached a screenshot.
Please fix this problem,

Are you having trouble with your form "GreenSocials - Order Youtube Packages"?
I did check your form but could not find any issue with that. It should not send any blank submission emails. Since I could not test your form as it has PayPal question, I cloned your form and did send some test submissions. The submission emails I received does have all the question as expected.
Do you actually receive blank email for every submission you receive or its just few of them?
It seems you are using thunderbird email client. I would suggest you to sign in to your gmail online account and see if there you see the submission emails with answers.
Do get back to us if the gmail also displays the blank submission emails.

No, they're just a few, maybe 3 in every 50 submissions I get, but it's still a significant inconvenience.
I opened my gmail account in firefox and I still see the completely blank submission emails.

Please contact with your email provider and request them to add JotMail's IP Addresses to their whitelist, this might help in this regard.
You can try another temporary workaround meanwhile by checking the submissions details through a form Submissions page:

Whitelisting won't make a difference - Like I said, this is a problem that has started recently.
Here is the latest 2 blank forms and the corresponding paypal payment e-mail. This happens to the same customer (firstname.lastname@example.org) over and over again. THis customer is forced to send his order info again to me via skype, due to the blank order forms problem.

Also, I tried looking at the submissions page but both blank forms were missing.

Is this happening only to this user? I just checked the form's email notifier and I see no problem with that. All of the fields are included in the email body.
Do you have this form embedded in a website? If so, can you please share the URL with us?

I encounter the same issue.
There is always a blank page, doesn't matter if I want to email the form or if its on our facebook page.

Do you mean to say that all submission emails you receive comes blank or only few of them? I did check your form's notification email template and it does have all the questions. It should send you the submission emails with all the answers.
