I need assistance please, my form has stopped being able to submit. it takes me to a white screen and nothing is sent

When I test your form, I was able to submit it without any problem.
Did you embed the form to your website? If yes, let us know the website URL so we can check the form here.
May I also suggest to clear your browser's cache and try to submit the form again. See if it is helping.

its not hosted on a site its hosted on an html file within an app and its still not functioning where can i send you the html file for review, although the html i havent changed at all and it was working up until 2 hours ago

there is the html code, although i have looked through it and there is nothing i can find as to why it isnt sending

i am beginning to think its not on my end cause with no changes made it started submitting forms partially and then stopped. is there any way i can get a credit for some of these forms ive submitted for testing?

It took me a while to figure out the problem because you are modifying your form source code. I hope the following will help.
You have added two textarea fields on your form source code which are not defined on your form in the form builder and this is causing the form to break.
Removing the following two textarea should resolve the resolve the problem.
I also noticed that you have modified some of the field names on your form which is not right. We do not recommend it since it will stop the form from capturing the data on these fields during the form submission.
I have also lower down your submission counts as I can see few test submissions on your form.

those areas are being used though they are populated using jquery and have always worked can you assist me on getting them to show on the form

is there anyway to revert to my form in the state it was yesterday? i cannot change settings that im sure you would be able to

Sorry but we do not commit or made any changes specific to user forms without any purpose. You have modified your form source code which is most probably the reason why the issue occurs. If you check your stand alone form in the form builder, it is working normally without the mentioned problem.
The best way to resolve the problem is to add two textarea fields to your form in the form builder. Preview your form and take the names of the two textarea fields from the source code and replace the textarea names on your existing form source code. Check the following screens.
1. Add textarea
2. And then copy the field names of the two textarea fields and replace the one on your modified form source code.
