I haven't gotten any form responses in 5 days (and I tried it myself), and the form will not submit. Please fix. I am losing business.Page URL:

I cloned your form to test is and everything seems to be working as far as the setup. After submission I get redirected to this page.
I also checked your form limits and everything seems to be ok also. The problem appears to be with the widget you are using for the form submission. I have escalated your ticket to see if this is a bug with the widget. In the mean time if you remove the widget, the form works correctly.

I see this form appears to be receiving submissions just fine now. Please let us know if you are still encountering issues with receiving submissions.
