    Please can you check that my form: Pure Democracy Party - Membership Form is correctly set up?

     

    I am not sure it is properly set-up.  Please can you check?

    I cloned your form to test it and everything seems to be working fine.  After filling it out and submitting, I am redirected to your payment page:

    Since I did not send payment, I also receive the notice of incomplete payment in the view submissions tab of my cloned form:
