I have tested your form directly and also by cloning your form. It appears that the Custom URL is not redirecting, not to your URL or any of the ones I have tested. I will escalate this ticket to the Dev Team so that they can review further what could cause this. Once we have any updates, you will be notified through this thread.

There was an old, invalid condition rule in your form. When I disabled it, your form worked as expected. We are truly sorry for the trouble caused to you and grateful for your report.
