What is exactly your service?

I pay for subscription. I already have my survey. I would like to know your service.  Do you provide your page?

 If you provide the page I would like to know the disposal process.

I am not sure if I have understood your question correctly.

You can create your form using our service and embed it in your own web page. You can also share your form's direct URL of your survey so that people can fill your survey form and submit.
