I use the image wizard and the field contains a three lines in the shape of a triangle pointing toward the lower right corner. When I apply the image size through the properties menu, the image appears to be broken
I checked your form: http://www.jotformpro.com/form/41106108228949 and noticed that you have successfully added an image below the header part.
After adding the image field, you can close the wizard and just paste the image link to the Image Source area - found in the image properties:
Just please make sure that you have full image URL into that area.
Can you give us an example that shows the broken image? This way, we can check for the possible cause and help you fix it.
