After testing the form and after clients submitting the form, the info I receive via email is this: "Undefined"Page URL:

Have you embedded the form to a website? Or, are you using the form direct link (custom URL) ? If the form is embedded, then the most possible reason is a conflict between your website scripts and the form scripts. Re-embedding your form using our iFrame method should help you fix it.
If you are using the form direct link, then I would advise to just re-save your form into our form builder. After that, clear your forms cache by following this guide: http://www.jotform.me/help/229-How-to-Clear-your-Form-Cache.
