From your question I understood that you are trying to clone a pdf form in order to use it. Is this correct?

I'm not sure if the clone option accepts pdf files. The clone option is meant to clone website version forms, so I believe it is necessary to manually create this form.

In alternative you can browse our template gallery to see if you can find some form similar to this.

Please let us know if further support is needed.

    Thank you

Thank you for your response. No, I wasn't trying to clone the pdf. There are two options, to pull up a form using a url or to clone the document. I was trying to use the url shown above in my initial question.

As far as cloning, would I only be able to use that for an already existing Jotform document? I'd like to use the form we have already created, the one that is linked to through the url above. Please help!

The url you shared is from a pdf document even though it is online:

If trying to clone a form from Jotform it's easier as we are talking about the same code. It is possible to clone other forms but sometimes it happens some minor changes in the final form.

Can you export the above form in a different format? Maybe we can find an alternative solution.

    Thank you

What type of format would work?

If html is an option it could be good to give it a try.

But honestly I believe the best option might be trying to find a template that fits your needs and then adapting the model.
