My form is not showing up in my account

Hi I created this form last week, but it's not showing up in my account.

Hi,

The form was created with a guest account.  You may not have been logged in when you made it and the form must not have been attached to your account properly.  I went ahead and moved it to the account you logged into at "joshbrown".  Please let us know if there is anything else we can assist you with and we will be happy to help.
